IndiGo, India's largest airline, experienced massive operational disruptions and nearly 4,500 flight cancellations between December 1 and 9, 2025, causing widespread passenger distress. In response, IndiGo's board approved the appointment of Chief Aviation Advisors LLC, led by aviation veteran Captain John Illson, to conduct an independent root-cause analysis of the disruptions and recommend improvements. The India — Directorate General of Civil Aviation (India) (DGCA) is also investigating the crisis, questioning IndiGo CEO Pieter Elbers and COO Isidre Porqueras, and has suspended four Flight Operations Inspectors over operational lapses.
