A study published in The Journal of Nuclear Medicine by researchers led by Masaki Shiota of Kyushu University demonstrates that a noninvasive circulating tumor DNA (ctDNA) blood test can identify metastatic castration-resistant prostate cancer (mCRPC) patients most likely to benefit from 223Ra radiopharmaceutical therapy and monitor treatment response. The study analyzed 93 mCRPC patients using an 88-gene panel before and after 223Ra therapy, finding that higher ctDNA levels and specific gene alterations (TP53, PTEN, cell cycle pathway) were associated with worse outcomes. The test could refine patient selection and enable early detection of treatment resistance, but requires external validation and prospective studies before insurance coverage.
